Case Manager - AFC/Shared Living - Full Time

Pay $24.70 / Hourly

SUMMARY The Care Manager assumes primary responsibility for each member on an assigned caseload, including record maintenance and the provision of psychosocial and counseling assistance for the Adult Foster Care (AFC) program.

Adult Foster Care (AFC) is a program for individuals on MassHealth with chronic health care needs. Individuals in AFC need help with daily living skills but wish to continue living an active lifestyle in the community. The AFC program allows people to enjoy the comforts and atmosphere of a private home with assistance from a paid family or non-family caregiver and provides nursing and care management to support the family as well as the individual. The registered nurse and case manager schedule regular home visits to monitor the placement, as well as provide any necessary training the family may need.

ESSENTIAL FUNCTIONS:

  • Conduct initial and ongoing evaluations of the AFC qualified program setting.
  • Obtain social history and conduct psychosocial assessment and evaluation.
  • Select, train, evaluate, and supervise AFC caregivers in conjunction with program RN.
  • Participate in development, implementation, and ongoing review of the AFC Plan of Care.
  • Conduct on-site visits with each member at the qualified setting as regulated.
  • Complete progress notes, maintain program member charts, and review care logs as regulated.
  • Participate in interdisciplinary meetings; discharge planning, transition planning, and implementation.
  • Review and document the fire safety procedures for the qualified setting.
  • Asses the qualified setting annually.
  • Assist with obtaining information and access of health care and community services.
  • Perform other duties as assigned by the supervisor, division management team member, or division director.
  • Participate in all staff meetings and required training sessions, including in-service training, as scheduled.
  • Maintain all program and Agency required trainings.

QUALIFICATION:

  • Knowledge obtained through attainment of a bachelor’s degree from an accredited institution and two years of recent experience working with elders or adults with developmental disabilities
  • Skill in communicating and working both independently and as a team member
  • Ability to effectively participate in design, implementation, and coordination of services
  • Ability to complete and disseminate all required forms and documents
  • Ability to maintain a valid driver’s license and reliable transportation
  • A qualifying criminal background check, driving record, and valid driver’s license
