SUMMARY: The Respite Direct Care Professional (RDCP) works together as a team to provide Premier services to the individuals in our care. This position will: provide respite to an individual and relief for a family that includes but is not limited to individual outings, meal preparation, and ADL care if needed, distribute medication while maintaining human rights at all times. This position will implement Positive Behavior Support and Individual programs as identified in the Individual Service Plans. At times, de-escalation and crisis management techniques may be required to maintain the safety of others. This position may require a flexed schedule including 1st, 2nd and 3rd shifts. This position is required to work on a weekends.
ESSENTIAL FUNCTIONS:
1. Instruct and assist individuals with personal hygiene, housekeeping, social skills, leisure time use, advocacy, and response to emergency situations.
2. Plan for and provide support for community-based activities (as required) that are tailored to the individual’s strengths and interests.
3. Implement and document individual behavior programs where applicable.
4. Facilitate community integration by encouraging individual preference and providing opportunities for recreation activities.
5. Encourage and assist individuals to pursue relationships and develop rapport with local community resources such as stores, restaurants, etc.
6. Instruct and assist individuals with meal planning, preparation and grocery shopping; and encourage individual participation to the fullest extent with an emphasis on healthy choices.
7. Deal with immediate problems in the home; notify direct supervisor or the on-call supervisor.
8. Assist individuals with financial management to the degree specified in the Individual Service Plan; and manage petty cash, including documenting and providing receipts for all money spent.
9. Instruct and physically assist individuals with housekeeping activities when living in house for respite days; and promote a sense of pride and ownership in the home.
10. Ensure home is safe and free of clutter and debris; and instruct individuals in fire prevention and evacuation Procedures.
11. Participate in all staff meetings and required training sessions, including in-service training, as scheduled.
12. Provide individuals with transportation as needed.
13. Assist and/or support individuals in any emergency.
14. Provide safe physical intervention of individuals as necessary.
15. Awake overnight positions are closely monitoring individuals, completing assignments and are awake at all times.
16. Respite services provided could be 24 hours.
17. Respite support services may require a one to one staffing.
