Intake Coordinator - Full Time

SUMMARY: The employee is responsible for the coordination of client referral, intake, and assessment prior to admission to the Early Intervention Program (EIP) and staff orientation activities.

ESSENTIAL FUNCTIONS:

• Coordinate the referral process with secretarial/billing staff and schedule all initial intakes and assessment for prospective clients.
• Ensure all families are entered into services with a signed IFSP within 45 days of referral.
• Maintain all pertinent records and statistics for program admission.
• Attends all required management meetings.
• Facilitate transition of referrals age 2-9 years to 3.0 years to appropriate educational program.
• Develops and implements the staff orientation program.
• Serve as a liaison between EIP and the community.
• Attends community meetings as directed.
• Maintain assigned productivity level on a monthly basis.

QUALIFICATION REQUIREMENTS:

• Knowledge obtained through attainment of a Bachelor degree (minimum) in Social Work, Nursing, or Counseling from an accredited institution, Master’s Level preparation is preferred.
• Knowledge and skill obtained through Pediatric/Early Intervention experience.
• Maintains on file, a current copy of all required professional licenses, CPR, first aid and health records.
• Skill in communicating and working with others.
• Ability to proactively participate in program planning and administration.
• A qualifying criminal background check, approved driving record and a valid driver’s license required.
